HDC Discover provides multiple ways to review and analyze encounter data so practices can turn information into action.

What Counts as an Encounter

HDC Discover includes encounters that meet specific criteria to ensure accuracy and consistency across EMRs. In general, we count encounters that were not cancelled, rebooked or no show.

More specifically, encounter definitions vary slightly by EMR:

  • MOIS: A contact (encounter) is counted when it:
    • Is signed off
    • Has a chart number
    • Has a status that is not N (No Show), C (Cancelled), or R (Rebooked)
  • OSCAR: An encounter is included when the encounter note has a state of 1 (signed)
  • Med Access: A visit with a status of “complete”
  • Profile: An encounter with a status of “resolved”

These criteria ensure that only valid, completed patient interactions are included in reporting, helping practices rely on consistent and meaningful encounter data.

Viewing Encounters in HDC Discover

  • Log in to HDC Discover
  • On your Dashboard, scroll down to view your encounter insights
  • Your Encounters in the past year: You can check your percentage of encounters with active patients recorded in the EMR in past year
    • Results are categorized by sex/gender and age rage.
  • Encounter with Panel Patients: The percentage of encounters of a family physician or nurse practitioner has had with their panel patients in the past year
    • This reflects how often a family physician or nurse practitioner attends to their designated patient panel.

  • Navigate to the QI Panel
  • Click the Patient Medical Home domain

The Patient Medical Home domain contains a comprehensive list of encounter measures. Click on any measures to get more insights into your encounter data.
