HDC Discover is a web-based application that empowers primary care providers to recognize trends, identify strengths, and consider opportunities in their unique practices.
HDC aggregates health data to enable sharing and comparison of aggregate clinical measures, identified and defined as per the HDC’s Clinical Data Stewardship Committee (CDSC) that is composed of your primary care peers.

HDC Discover currently has over 350 measures and we are always working hard to add more relevant ones.
Measurement categories include disease management, prevalence, prevention, screening, documentation, and panel management. All measures are vetted by the CDSC during development to ensure physician stewardship and oversight.



Privacy First
Privacy is a guiding principle at HDC and is built into our services, infrastructure, policies, and processes. We build privacy into all aspects of our operations using the Privacy by Design Framework and are ISO 31700-1:2023 MSECB Certified. Learn more about our privacy first approach.
Use HDC Discover to:
- Better understand your own patient population and patterns of practice on its own or as it compares to your clinic and larger community.
- Explore data-informed learning based on your interests in a safe, secure environment that maintains confidentiality for you and your patients.
- Share and reflect on progress with colleagues, but only if you choose to.
- Inform valuable discussions with your practice coaching team in partnership with the Practice Support Program, as well as the peer mentoring program.
- Implement practice improvements and provide data-informed care to your patients.
Using HDC Discover is a visible sign of your professional commitment to clinical practice improvement, as well as contributing to valuable community or regional trends to inform your peers and supporting organizations, like Divisions of Family Practice.
