The Health Data Coalition supports the use of HDC Discover for Divisions of Family Practice, to enable communities to identify unique features, trends or opportunities to build stronger and healthier communities.
Divisions of Family Practice have played a key role in the evolution of the HDC, recognizing early on, how quantitative clinical measurements could demonstrate collective impact in stewarding resources for patients.

Divisions of Family Practice now have access to their own dashboard, which is designed to give division users a more complete and actionable view of activity across clinics in their division.
The goal is to provide a practical and easy-to-use tool that highlights key insights into clinic participation, patient access and attachment, and overall population health.

HDC Discover has added value to regional projects and programs, contributing to the improvement of patient outcomes.

Appointing Your Representative Member
The Health Data Coalition (HDC) enacted a new governance structure in June 2018, and each Division of Family Practice in BC, the Society of Specialists, and the Specialist Services’ Committee were invited to appoint a physician as their proxy to represent their member organization at the Annual General Meeting.
This membership provides Divisions and Specialist organizations with a seat at the table to elect the Board of Directors, vote on key issues, and inform the future development and usage of HDC data.
Each year, Divisions of Family Practice and Specialist organizations will be contacted to appoint a proxy or “Member Representative” to participate in the HDC’s Annual General Meeting.
