The Health Data Coalition (HDC) is a physician-led data sharing initiative that encourages self-reflection and collaboration with colleagues, while facilitating quality improvement in patient care through its application HDC Discover.
HDC is a not-for-profit organization funded by the Family Practice Services Committee (FPSC).

Our HDC Discover application provides access to a secure, core set of anonymized aggregate data, while maintaining both patient and physician confidentiality.
HDC was established in April 2016 through a merger of AMCARE and the Physician’s Data Collaborative to support a network of physicians for the collaborative use of clinical data.

Vision and Purpose
As a Family Practice Services Committee (FPSC) funded initiative, HDC is committed to building collaborative relationships with our mutual partners to collectively strive to achieve our Vision of a learning culture where primary care data is transformed into applied knowledge. Our work is patient-centred; the ultimate value of our efforts will be seen in improved outcomes for patients.
HDC continues to build a network for collaborative use of aggregated measures from clinical data that is captured within primary care Electronic Medical Records (EMRs) to support Family Physicians (FPs) and Nurse Practitioners (NPs) to understand their practice measures, as well as guide and support professional and personal meaning in work.HDC enables data-informed patient care through practice improvement to support the Institute for Healthcare Improvement’s Quintuple Aim framework: improving the patient experience of care (including quality and satisfaction), improving the health of populations, reducing the per capita cost of health care, improving the work life of healthcare providers, and advancing health equity. HDC also supports the aim of optimizing the provider experience by supporting learning and practice improvement within a community of trusted colleagues.
