Heart Failure with Reduced Ejection Fraction (HFrEF): We have added the following measures to support QI projects related to HFrEF.

HFrEF and prescribed three or more of the four foundational medication classes in the past year  NEW  NEW  NEW  NEW  NEW

Improvements

Top 10 Coded Prevalence: To improve the accuracy of this list we have excluded conditions where there were overlapping definitions. For example, the “depressive disorder prevalence” and “bipolar disorder prevalence” are subsets of “mood disorder prevalence”. The prevalence measures that are included in the Top 10 Coded Prevalence list are:

Anxiety and phobias prevalence  Diabetes prevalence  Multiple sclerosis prevalence
Asthma prevalence  Eating disorders prevalence  Neurodevelopment disorders prevalence
Bipolar disorder prevalence  Frailty prevalence  Osteoarthritis prevalence
Cardiovascular disease prevalence  Heart failure prevalence  Palliative/end of life prevalence
Chronic kidney disease prevalence  Hepatitis C prevalence  Parkinson’s disease prevalence
Chronic liver disease prevalence  HIV prevalence  Primarily psychotic disorders prevalence
Chronic pain prevalence  Hypertension prevalence  PTSD prevalence
COPD prevalence  Hypothyroidism prevalence  Rheumatoid arthritis prevalence
Cystic fibrosis prevalence  Inflammatory bowel disease prevalence  Sleep disorders prevalence
Dementia and cognitive impairment prevalence  Ischemic cerebrovascular disease prevalence  Substance use disorder prevalence
Depressive disorder prevalence  Ischemic heart disease prevalence

Updates to Conditions Domain We have added a “Metabolic Syndrome” subdomain to the “Conditions” domain to support the accessibility of this new measure topic.

“Nonbinary or Unspecified” Definition Added: To improve the clarity of our measures that have an “nonbinary or unspecified” option, we have added the following definition to the “Concepts & Logic” section:

“Where nonbinary or unspecified refers to a patient whose sex or gender is not recorded as male or female in the EMR. This includes entries such as T, O, U, X, I, Unknown, Other, or a blank value in the Sex or Gender field.”

 Related Measures:

  • Active patients – nonbinary or unspecified
  • Encounters in the past year – nonbinary or unspecified
  • Encounters in the past 3 years – nonbinary or unspecified

Concept Update

To improve the accuracy of our measures, we have made changes to the following concepts:

Concept  Changes  Impacted Measures
Mental Health  Added “Neurodevelopment Disorder” to the conditions included in this concept  OUD and mental health diagnosis
