HDC Discover can support learning for individual physicians, nurse practitioners or groups of providers within a Primary Care Network or a Division of Family Practice. The HDC team is here to support you and are available to provide facilitation support or to work with a Division team to develop a learning program specific to your needs.
In addition, one-on-one orientation sessions are always available to help not only Division staff but also community physicians better understand their clinical trends in HDC Discover.

These webinar programs were designed to help increase understanding of the practical clinical use of the HDC Discover application. Each of our webinars focus on a different disease area or clinical problem, and can also serve as a ‘primer’for participation in PSP practice improvement activities.
To understand how to utilize HDC Discover in a clinical case-based quality improvement project within your practice.
To be able to interpret your own practice patterns, for instance in COPD and Asthma, and relate to your peers and provincial trends.
To be able to undertake QI projects either with PSP (supported by a PSP coach, mentor), or independently using a tool such as HDC Discover.

To help engage your community physicians in using HDC, the GPSC is now offering a one time payment for completion of the panel management program along with registration and orientation in HDC Discover. This payment is the latest step the GPSC is taking to support family doctors. For more information, visit GPSC’s website.

Community-level quality improvement
As part of GPSCs ongoing efforts to improve data-informed care at both the practice and community level, HDC isworking with Divisions of Family Practice to capture how HDC Discover is helping communities and how we can further support Divisions as PCNs evolve.
Is your Division interested in or currently working on initiatives to address specific areas of primary care, such as patients with mental health and substance use challenges, COPD, or diabetes? With data insights from HDC Discover, Divisions can create quality improvement goals that are the most relevant to their Division, based on physicians’ EMR anonymized data, conveniently aggregated to a single view.
