HDC Discover can support learning for individual physicians, nurse practitioners or groups of providers within a Primary Care Network or a Division of Family Practice. The HDC team is here to support you and are available to provide facilitation support or to work with a Division team to develop a learning program specific to your needs.
In addition, one-on-one orientation sessions are always available to help not only Division staff but also community physicians better understand their clinical trends in HDC Discover.

Community-level quality improvement
As part of FPSCs ongoing efforts to improve data-informed care at both the practice and community level, HDC is working with Divisions of Family Practice to capture how HDC Discover is helping communities and how we can further support Divisions as PCNs evolve.
Is your Division interested in or currently working on initiatives to address specific areas of primary care, such as patients with mental health and substance use challenges, COPD, or diabetes? With data insights from HDC Discover, Divisions can create quality improvement goals that are the most relevant to their Division, based on physicians’ EMR anonymized data, conveniently aggregated to a single view.
