Ways to Get Involved:
- Data Reflection/Quality Improvement Projects: Participate in initiatives that improve your care delivery and patient outcomes
- Peer Learning: Join communities of practice and share insights with colleagues
- Data-Driven Planning: Use aggregated data to inform clinic-level and community-level resource allocation and decision making.

Use of HDC Discover is offered at no cost to you or your practice, thanks to the funding of the Family Practice Services Committee (FPSC).
Access for Nurse Practitioners is provided via a service agreement with the Nurse and Nurse Practitioners Association of BC (NNPBC).
Online registration takes only a few minutes.
- HDC will work with your clinic staff to ensure a smooth registration process and get your data flowing.
- We will walk you through the registration process
- After initial setup, ongoing data collection and transfer is automatic, with data being uploaded every 3 months.
